A: 3 or 4, probably
There are certainly known undecidable statements in Peano arithmetic. Godel proved that one quite brilliantly. But your question isn't whether they are any; your question is what is the shortest one. That's a tough question, and one that I don't think mathematicians have the answer for yet.
The shortest peano statement I could find whose answer we do not know is:
.
As it's not known currently whether the number of n^2 - 2 primes is infinite, we don't know whether this statement is true or false. It's possible that we cannot know and that this statement is undecidable, but I can't prove that (and if you could, I'd be very much interested to hear about it).
The real answer isn't known. I don't know of any proofs of undecidability that are shorter than Godel's, but Godel's is really, really, really freakin' huge, so it's a near-certainty that there exist very short ones. Sorry I couldn't answer your question better. My guess is that the shortest such statement will have 3 variables, but that's just a guess.
